  • Individuals must be eligible to receive free services by meeting income eligibility requirements, and age requirements. Individuals are advised to call for additional information.

To be eligible individuals must: Reside in NY, be uninsured or have health insurance, but with a cost share that creates a barrier to obtaining screening and/or diagnostic services. If an attendant is not available when you call, individuals may leave a message and your call will be returned promptly.
